We ask that all patients continue to complete a survey prior to their appointment to reduce the risk of transmission of the virus. Below is a link to the questionnaire.

We now offer online forms! Filling out the registration forms before your appointment will reduce your time in the waiting room. Below is the link for our forms. IPads are available in the office if you cannot fill out the forms at home.

1. Medical history and registration forms are available online. When possible, we encourage patients to complete these forms prior to their appointment so you can be seen for treatment on time. You will receive an email with a link when you schedule your appointment.
2. Refreshments, magazines, and coloring books continue to be absent from the waiting area at this time.
3. Below is a link if you plan to pay with CareCredit. You may pay a balance directly through the link if you prefer.
